Όταν ήταν ακόμα έφηβη είχε δηλώσει στη μητέρα της πως είναι [[Αμφιφυλοφιλία|pansexual]] (δηλαδή έλκεται ταυτόχρονα και από τα δύο φύλα). Είχε δηλώσει χαρακτηριστικά: ''"Ποτέ δεν έβαζα στον εαυτό μου ταμπέλες. Είμαι έτοιμη να αγαπήσω οποιονδήποτε με αγαπήσει για αυτό που είμαι!".'' Επίσης, είναι γνωστό ότι η ίδια είναι και υποστηρίκτρια της [[ΛΟΑΤ|LGBTQ]] κοινότητας.
 
Το 2009, στα γυρίσματα της ταινίας "[[Το Τελευταίο Τραγούδι|The Last Song]]", γνώρισε τον ηθοποιό Λίαμ Χέμσγουορθ και σύντομα έγιναν ζευγάρι. Στα μέσα του 2011 αρραβωνιάστηκαν αλλά το 2013 χώρισαν. Επανασυνδέθηκαν τρία χρόνια μετά και στις 23 Δεκεμβρίου 2018 παντρεύτηκαν. Τον Φεβρουάριο του 2019, ο Χέμσγουορθ αποκάλυψε σε συνέντευξη του ότι μετά το γάμο τους, η Σάιρους άλλαξε το ονοματεπώνυμο της σε Μάιλι Ρέι Χέμσγουορθ.
 
Όσο δεν ήταν ζευγάρι με τον Χέμσγουορθ, έγιναν γνωστές δύο σχέσεις της. Αυτή με το μοντέλο Στέλλα Μάξγουελ, η οποία κράτησε λίγους μήνες του 2015 και αυτή με τον ηθοποιό Πάτρικ Σβαρτζενέγκερ, από τα τέλη του 2015 μέχρι τα μέσα του 2016.
